Professional Service Agreement

A Professional Service Agreement (PSA) is used for a vendor/contractor that is not an employee of the University of Montevallo, who provides infrequent services for a specific period of time. Once you have filled out the Agreement, you will need to complete the following:

  1. Once you have selected a vendor, complete the Universities portion of the agreement (i.e. description of services, budget, UM contact)
  2. Send to vendor for personal information and signature.
  3. Once returned from vendor, send the signed agreement to the General Counsel at contracts@2fitfashion.com.
  4. Once returned from Business Affairs, fill out a Request for Payment Form, attach the signed PSA, and send to Accounts Payable at Station 6060 or ap@2fitfashion.com.
