Professional Service Agreement
A Professional Service Agreement (PSA) is used for a vendor/contractor that is not an employee of the University of Montevallo, who provides infrequent services for a specific period of time. Once you have filled out the Agreement, you will need to complete the following:
- Once you have selected a vendor, complete the Universities portion of the agreement (i.e. description of services, budget, UM contact)
- Send to vendor for personal information and signature.
- Once returned from vendor, send the signed agreement to the General Counsel at contracts@2fitfashion.com.
- Once returned from Business Affairs, fill out a Request for Payment Form, attach the signed PSA, and send to Accounts Payable at Station 6060 or ap@2fitfashion.com.
